Nurse Practitioner RN (EC) – Part-Time Brantford Site

(1.0 Part-time [20 hrs/week])

The Delhi Community Health Centre (DCHC) is a community based not-for-profit organization dedicated to providing quality, patient-centered health care for individuals and families residing in Delhi and surrounding communities.

A variety of health care services and providers are available through the Centre. Housed within the Centre, the Delhi Family Health Team (FHT) provides access to family physicians, a nurse practitioner, registered nurses, social worker, mental health workers and pharmacist. A community Walk-In Clinic is offered and, also available, are additional family physicians and specialists who provide care via their private practices.

Supporting individuals who are accessing their primary healthcare through the Centre, in addition to supporting the broader community, a pharmacy, community lab and ultrasound services are available on site, as are a variety of other community services intended to enhance the overall health and wellness of men, women, children and families.

A secondary site can be found in Port Rowan.

The FHT has recently received funding from the Ministry of Health to launch a Chronic Non-Cancer Pain Management Program.  This program will serve Haldimand-Norfolk and Brant Sub-regions of the Hamilton Niagara Haldimand Brant Local Health Integrated Network (HNHB LHIN), with sites in Delhi, Dunnville and Brantford and will provide interprofessional primary care interventions to support individuals with reducing their prescription opioid use.

The interprofessional primary care team will be comprised of Nurse Practitioners, Registered Nurses/Registered Practical Nurses, Social Workers and Physiotherapists, who have expertise in pain management.  This team will be working along side of a variety of physicians who specialize in specific areas of pain management and opioid alternative therapies.

An immediate need exists for one part-time Nurse Practitioner.  This part-time Nurse Practitioner will be working at the Brantford site of the CNCPMP.


General Overview:

The Nurse Practitioner RN (EC) with the Chronic Non-Cancer Pain Management Program (CNCPMP) works along side of the pain management physicians and interprofessional primary care team, providing assessment, care and treatment planning as it relates to patient pain and management of the same.

 Responsibilities:  The Nurse Practitioner

  • is responsible for providing assessment and monitoring of patients who have historically been prescribed opioids for management of chronic non-cancer pain and are undergoing opioid titration or engaging in alternative pain management therapies.
  • is responsible for prescribing medications, in consultation with the pain management physicians and in accordance with the patient care plan and associated goals.
  • is responsible for assessing potential side effects from prescribed medications and/or long term opioid use, ordering relevant diagnostic tests and diagnosing complications.
  • is responsible for liaising with and referring back to the patients’ primary care physicians (the original referring physician) for non-pain related health concerns.
  • reports to the Executive Director and works closely with other members of the interprofessional primary care team.


  • Undergraduate or Masters Degree in Nursing
  • Valid certificate of registration in the Extended Class from the College of Nurses of Ontario and eligible to practice as a Nurse Practitioner
  • Current member of Nurse Practitioners’ Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO)
  • Successful completion of College approved controlled substances education in Ontario and entitled to practice with no restrictions
  • Current certification and maintenance of CPR for Health Care Providers
  • A minimum of two years’ experience working with individuals living with chronic pain
  • Knowledge of opioid de-prescribing strategies and alternative pain management therapies
  • Experience in chronic disease management and knowledge of patient self-management strategies
  • Strong clinical, organizational and interpersonal skills

  • Multilingualism (e.g., French, Polish, Hungarian, Low German) an asset
